Payment in full is due for each treatment provided at the time it is performed. For cases involving “in-network” insurance plans, the appropriate co-payment is due at the time of service. Our office is “in-network” with Delta Premier and PPO, Guardian PPO, Cigna PPO, Aetna PPO and Assurant PPO. We are “out-of-network” with all other insurance plans. As a courtesy to our patients with “out-of-network” dental benefit plans, we will submit necessary claim forms, receipts, and other information to your insurance company to help you receive the maximum reimbursement for any treatment we provide.
